OUR MISSION

BLOOM FAMILY HOME handles all daily housekeeping tasks throughout the house, including your room. Flat linens are laundered on a regular schedule, or sooner if necessary. Additionally, care staff regularly wash and fold your personal laundry.

We provide a variety of social and recreational activities each week to ensure residents remain engaged, entertained, and occupied. Don't hesitate to inquire about this week's scheduled activities.

The base price includes all standard household utilities, except for personal cell phone plans, which are the responsibility of the resident or their family.

BLOOM FAMILY HOMES welcomes Medicaid recipients and is currently looking for a Medicaid and Private resident for an available room. To qualify, Medicaid recipients typically need an assessment that evaluates their required level of care and the daily and monthly amounts covered by Medicaid for their services.

Certain waivers may also be accessible for eligible residents, potentially covering additional items not included under Medicaid. Keep in mind that Medicaid regulations, coverage, limits, and waiver programs vary significantly by state. If you haven't done so yet, please reach out to your local Medicaid office to find out how to obtain an official assessment and determine your eligibility for any additional coverage.

Once you have this information, you can contact our facility (or any facility that accepts Medicaid), and we will ask you questions regarding your care needs and Medicaid assessment. The facility will then make the final decision on whether to accept the resident.

EATING - A weekly schedule for home-cooked meals with alternative options will be provided. Individual diet and nutritional preferences are considered. Reminders to full feeding will be available. Eating assistance and guidance will be provided. 

TOILETING - From cueing and monitoring to full assist. A standard toilet with two adjustable grab bars on both sides are installed. 

WALKING - From cueing and monitoring to full assist. Walking outside is always encouraged and will be accompanied. 

TRANSFERRING - We assist with bed, chair, and wheelchair lifts, and getting in and out of vehicles for transport. A Hoyer will be available to the Residents needing it. Assistance from cueing and monitoring to full physical transfers. 

POSITIONING - This can be done as required and needed. 

PERSONAL HYGIENE - Full assistance available. Personal items, such as dentures and hearing aids are cleaned as needed. 

DRESSING - From cueing and monitoring to full assist. If possible, Residents can make their own decisions on daily wear. 

BATHING - Bed Baths or full showers are available. Cueing and monitoring to full assist. A walk-in shower is available and is wheelchair accessible. 

 We administer medication per doctor's order and nurse delegation. Each Resident’s needs are reviewed and approved through the Nurse delegation RN.

 Skilled nursing is provided through a nurse delegation contract—the ability to delegate nursing tasks as needed within a reasonable time. 

 Our staff is certified and meets all State requirements. Our specialty care includes: 

• Mental Illness 
• Dementia 

 The Resident Manager lives in the home. Certified Nursing Assistants and Caregivers are available 24 hours and 7 days a week. A call button is available in each Resident’s room.  

 Our home serves meals that accommodate cultural and ethnic backgrounds and provide informational materials in a language understood by Residents. We are fluent in English and Tagalog and understand some Spanish, and will work with Residents to communicate effectively. 

 Our home will accommodate Medicaid payments with notice of changes. 

 We are open to any ideas. Our home enjoys music, classical movies, and nature. We also offer the following activities:
